Nancy Buchholz, APNP - ThedaCare Physicians-Black Creek

Kristin Collar is a board certified nurse practitioner who specializes in family medicine. She went into family medicine so she could care for a variety of patients and their families, from birth to end of life. Her passion lies within preventive health maintenance, with a special focus on women's health. Education: Bachelor's degree in Nursing from Bellin College of Nursing, 2006 Master's degree in Nursing with certification by the American Academy of Nurse Practitioners as a Family Practice Nurse Practitioner from UW-Oshkosh, 2011 Kristin enjoys spending time with her husband, daughter, and yellow lab. She also likes to camp at state parks and visit her parents in Florida.
