Together We Can Foundation-Smart Transitions is a 501 c 3 charity based in Virginia Beach, VA, dedicated to improving outcomes for at-risk youth in transition to independent adult life and the workforce in Hampton Roads and Southeastern Virginia. They offer success skills classes and resources, partnering with local organizations to provide youth classes, resources, and staff training in various areas such as career identification, financial planning, interviewing skills, and leadership development.

The foundation has served over 8,000 youth since 2010, with a focus on fostering connections, optimism, and preparedness among foster youth, court-involved youth, homeless youth, economically disadvantaged youth, and others struggling with the transition to adulthood. Through their Life-Work Portfolio Course and Smart Transitions Guides, they aim to empower young adults to confidently navigate their future paths and achieve success in both their personal and professional lives.
