Tried to schedule appointment for mammogram but was refused because I could not provide physicians physical address. Would not set appointment unless address was given before coming in to facility.

Office hour is 8am to 5pm. I was there at 4:30pm as walk-in, but was told they won't take anymore for the day! There is nowhere I. The office or on their website showing this "Roll".
