Poor customer service - male employee /nurse dresses unprofessionally and is rude. Clearly there to just collect a paycheck.

Never again will this location be used by more and hopefully anyone else. First off the male employee or nurse doesn't care enough to even get your name or Sex right word of mouth or paper work....
