The group was started to meet the need for aging seniors in the community. With a busy geriatric practice located on the Grand Concourse, we realized that some of our patients were not doing better despite best medical care and medicines. This brought about the realization of the unique needs and barriers to care in the elderly. Most health care problems of elderly are chronic and debilitating. Seniors, juggling complicated drug regimes and medical issues, also have to deal with increasingly complex health care environment. This includes cost of transportation, prior authorizations, Medicare prescription drug plan, brand versus generic to name a few. Moreover, hospital discharge patients sooner resulting in sicker people at home. Home visit was a natural fit to overcome these barriers.
