At CHW Cares, the team of primary care, dental, and specialty providers specialize in providing comprehensive physical health and dental care to older adults. We go beyond treatment by teaching patients to identify danger signs before they become serious complications.CHW Cares currently serves the Washington Heights and Inwood neighborhoods of Manhattan, and is growing throughout New York City. Individuals aged 65 and older can access a comprehensive suite of health services, including asthma care and pulmonology, diabetes care, kidney care, gynecology, pain management, podiatry, and orthopedics.The team also offers all-inclusive cardiology care, including treatment of hypertension, venous ulcers, and varicose veins, nutrition counseling, physical therapy and rehabilitation, and thyroid disorders.CHW Cares provides eye care, neurology, and treatment for sleep disorders. Not to mention preventive and general dentistry, including root canals, teeth cleaning, teeth whitening, and fillings. For people who experience pain or anxiety, the team also offers acupuncture.Our care teams take time to meet with and understand each individuals symptoms and concerns. Our Primary Care Physicians collaborates with a team of health coaches, dieticians, nurses to support each patient on their care journey. We are committed to providing state-of-the-art medical care at home. We provide seamless continuum of care for seniors' citizens who are home-bound or unable to visit a doctor in office. The group was started to meet the need for aging seniors in the community. With a busy geriatric practice located on the Grand Concourse, we realized that some of our patients were not doing better despite best medical care and medicines. This brought about the realization of the unique needs and barriers to care in the elderly. Most health care problems of elderly are chronic and debilitating. Seniors, juggling complicated drug regimes and medical issues, also have to deal with increasingly complex health care environment. This includes cost of transportation, prior authorizations, Medicare prescription drug plan, brand versus generic to name a few. Moreover, hospital discharge patients sooner resulting in sicker people at home. Home visit was a natural fit to overcome these barriers. Complete assessment, dose adjustment, compliance with medicines and diet is best delivered in a convenient and comfortable environment. Hence we choose house calls for medical services House Call Medical Services are currently provided by a team of two physicians, a physician assistant and key support staff. Founded in June 2005 with just one provider, our group has grown exponentially due to need of our services and quality of care provided. We are providing over 160 visits a month in the Bronx and Lower Westchester area. In addition, we coordinate specialty referrals including home visits by podiatrist and wound care team. Blood test and diagnostic X-rays are provided at the convenience of home. Prescription refill including home deliveries are coordinated by our staff. We coordinate the medical services with all members of team including the family members, visiting nurse, home health aid, social worker and physical therapist. Our goal is to maintain our patient's ability to live independently, with best quality of life and eliminate the need for nursing home care or hospitalization.
