Pathways Community Hospice opened its doors to the St. Louis area with a mission to comfort patients suffering from serious illness and attend to their loved ones' needs with support and education. In 1998, we expanded upon this mission by joining the Delmar Gardens family. In 2008, we added a community-based palliative care program. Pathways Palliative Care is a nurse practitioner-based service that provides comfort to individuals who face chronic/life-limiting illness but are not ready or do not choose hospice care.
