Chosen Valley Care Center Inc. is a retirement home which offers residential options and supportive services. Based in Chatfield, Minn., it provides living options, such as the health care center, independent living, assisted living, along with the Adult Day Respite service. The center caters to the needs of seniors with nursing, long term care, post-acute rehabilitation, physical, occupational and speech therapies, recreational activities and daily meals with provisions for special diets. It provides special apartments with various amenities, on a rental basis, for independent or assisted living options.
