Since 1968, we've been serving individuals, families, and communities in the Twin Cities. Our nonprofit organization relies on the generosity of donors to continue offering high-quality services, including independent living, skilled nursing, and memory care. We strive to create a welcoming environment where everyone feels valued and loved.

Sholom Home West is a 170-plus-bed facility that offers a range of long-term and transitional care solutions. The facility maintains a memory loss and dementia care unit. It also provides various rehabilitative, adult day, and respite and hospice care services. Sholom Home West offers specialized medical care and pain management solutions. It operates a library, gift shop, barbershop and garden courtyard. The facility offers physical, occupational and speech therapies. Sholom Home West maintains private and semiprivate rooms, as well as provides psychological consultation solutions. In addition, the facility offers on-site podiatry, optometric, dental and audiology services. It is operated by Sholom Community Alliance, which is a nonprofit organization.
