Brooks Rehabilitation at World Golf Village provides skilled physical therapy services to meet your rehabilitation needs. Our experienced therapy staff provides the highest quality care in our 4500 sq. ft. facility which includes a therapy pool with an underwater treadmill. Upon entering the main entrance of the World Golf Village take the first left onto West Town Place. We are in the second building to the left on the first floor.

We moved! Our new address is: 104 Ashourian Avenue Suite 105 St. Augustine, FL 32092 The new St. Johns location of Brooks Rehabilitation Outpatient Services is located just east of St. Johns Parkway at the Forum. Our new location provides comprehensive services including physical therapy, occupational therapy and speech therapy for children and physical therapy for adults.
