River House Adult Day Center

Located in Norwalk, Conn., ElderHouse is a nonprofit agency that is funded by the United Way and the Southwestern Connecticut Agency. It offers nursing care and assessment, medication administration, personal care assistance and podiatry services. The agency also provides meals and dietary monitoring, counseling, caregiver respite and transportation services. Its nursing services include weight and blood monitoring, medication administration and glucose testing. ElderHouse additionally provides supervision and private aid, as well as grooming, dressing, toileting, hair care and walking assistance services for clients. The agency organizes a range of small group activities, such as music, arts and crafts, gardening and cooking.

Community Access Unlimited is a Union County-based nonprofit that serves people with disabilities and at-risk youth across New Jersey.
